Record Keeping & Project Report Forms
Record keeping and Project Reporting are valuable skills taught to members of the Kansas 4-H program. Members are encouraged to keep a permanent record of all 4-H and Non-4-H Experience and are encouraged to summarize their experiences on the Kansas 4-H Project Report Form for project recognition.
4-H Explained: Record Books
These forms are due in the Marais des Cygnes District each October 1st to be recognized for your Project Work. 4-H members who are chosen County Champion in their project and are 14 –19 by January 1st can send their Project Report Form to Regional Competition and compete against other counties in our area. If you are one of the two books chosen from the Regional Screening, you are eligible to send your Project Report Form to state . Winners of the state Project Records Competition are recognized at the Emerald Circle Banquet and have the opportunity to attend National 4-H Congress in Atlanta, GA.

A Record Book Consists of the following in Marais des Cygnes District 4-H.
- Marais des Cygnes 4-H Record book Check List (pdf)
- Kansas 4-H Personal Information Page (formfillable pdf)
- Kansas 4-H Project Report Form (Zsuites Online or PDF Form Fillable) See below for more information
These 3 items should be put in a Folder with brads, record book cover, or binder and submitted to either office by October 1st.

You can use Zsuites to fill out your Project Report Forms for your project(s) and your Permanent Record.
You will still need to download your Kansas 4-H Personal Information Page and your MARAIS DES CYGNES 4-H RECORD BOOK CHECK LIST to complete your record book.

Before you begin to keep project record reports for the year, please make sure you have a current
version of Adobe Reader DC. If you do not, you may download a free version, here:https://get.adobe.com/reader/. When you click on that link, the following screen will appear. "Uncheck" the 3 boxes in the middle column and click on the link circled in green to begin your download. Follow the prompts to download and install Adobe Acrobat Reader DC.
